The cleft lip and palate is a congenital malformation affecting the primary palate (nose, upper lip, alveolar) and / or the secondary palate (hard and soft palate to the uvula).
There are three types of fissures: lip, palate or lip and palate. In addition these fissures can be unilateral (one side of the mouth) or bilateral (both sides of the mouth).
Before a cleft lip surgery it is necessary that an interdisciplinary team consisting of pediatrician, otolaryngologist, plastic surgeon, dentist, speech pathologist, genetic and psychologist evaluate the child.
The first surgery is usually done between 2 and 6 months of life to treat cleft lip and the second to repair the cleft palate, from 10 to 12 months of life. The unilateral cleft lip usually requires a single surgery, if it is a bilateral cleft will need more surgeries.
Between 7 and 9 years old the doctors will evaluate the need to place a bone graft in the area where the teeth will come out to maintain the stability of the jawbone.
If necessary, after the 15 years old you can perform a rhinoplasty to create a more balanced appearance and increased facial harmony.
The first surgery is done to create a functional palate and so that the facial bones, teeth and speech have a good development. The first surgery will also help in the development of the feeding capacity.
The number of surgeries depend on the severity of the cleft. Usually wait 6 months between each surgery to make way for the formation of a scar.
Children with cleft palate often receive orthodontic treatment and speech therapy. The timing of such treatments depends on each individual patient.
As a consequence a scar may remain. a short lip or an incompetent palate for remained not very mobile or short. There is also the possibility of oro-nasal fissures.
